January 29, 2026 (Nashville, TN) - ArchWell Health™, a national healthcare company providing an advanced model of primary care for adults 60+, announced today its partnership with Cleveland-based non-profit Collaboration Station to support its “Crisis, Chronic Health, and Acute Prevention (CCHAP) Meals on Wheels Program”.

Through CCHAP Meals on Wheels, Collaboration Station provides nutritious, home delivered and medically tailored meals to homebound seniors with chronic or acute health conditions or disabilities. The organization also works with caregivers and out-of-town relatives of seniors who may find themselves in a crisis or whose travel outside their home is limited or totally restricted.

As a partner, ArchWell Health is supporting these efforts by hosting an Open House at its Cleveland Jefferson Center located at 11702 Lorain Avenue. The event takes place on Thursday, January 29, from 2 p.m. to 4 p.m. and will feature a presentation by Collaboration Station’s Executive Director Cheryl Smith on heart-smart nutrition and tips for staying healthy on a budget.

In Ohio, 7.2% of seniors have trouble getting enough food, which is higher than the national average and a clear reminder of the growing need for reliable nutrition support. The CCHAP program addresses this gap by improving seniors’ quality of life and expanding access to medically tailored, nutritious food.

"It is our pleasure to come alongside Archwell Health to serve senior populations across Cleveland and surrounding cities. As we enter our 10th year of serving the food insecure, we are excited about this new opportunity to expand our reach in addressing the food and nutrition needs of older adults at this crucial time," said Cheryl Smith, Executive Director, Collaboration Station.

"Collaboration Station’s commitment to serving seniors aligns perfectly with our mission at ArchWell Health, said Jamie Pointer, Regional Growth Manager at ArchWell Health. “By working together, we can help close the nutrition gap facing so many older adults in the Cleveland community while providing meaningful support when it’s needed most.”

ArchWell Health currently operates nine centers in the Cleveland metro areas including Brook Park, Eastlake, Euclid, Cleveland Heights, Maple Heights, Jefferson, Woodland Hills, Parma, and Westlake. Each center offers highly personalized primary care services with physicians and team members focused on helping members improve their health, reduce the effects of chronic diseases, and live healthier lives. Patient-to-doctor ratio is below the national average with doctors dedicating more time to every individual member, addressing their unique health challenges and questions. In addition to high-quality primary care, ArchWell Health also provides same-day appointments and weekly activities in a bright, welcoming environment that fosters friendships and connections.
