Cindy Rosales, Licensed Master Social Worker will be discussing the importance of Advanced Care Planning (ACP). Take part in an open question and answer session while enjoying snacks and refreshments. This event will answer questions like:

• Why do need to complete my Advanced Care Planning or document my DNR status?
• How can I get connected with resources in my community?
• Can ArchWell Health help me understand my options?
