Advanced Care Planning Workshop

Cindy Rosales, your center's Licensed Master Social Worker will be hosting an Advanced Care Planning Workshop to assist members with their care plans, 5 wishs, and other directives. Take part in a hands-on, open question and answer session while enjoying snack or refreshment. This event will answer questions like: 

• How do I make Advanced Directives or a Living Will?

• Can I get connected with resources in my communitiy?

• What is a DNR?

• Can I get help understanding my options?
